Parents or legal guardians must have all of the following information before a student may be legally enrolled:
 

1. Updated Georgia Immunization Certificate
2. Eye, Ear, and Dental Screening Certificate
3. Copy of child's birth certificate
4. If you are a child's legal guardian, pleas bring guardianship papers.
5. A current report card from the child's previous school with recent grades and indication whether he/she passed or failed.
6.Your child's Social Security card.
7. Proof that you are a current resident of Putnam County (any document indicating your current address such as an electric bill or phone bill).

Parents should be prepared to fill out registration packets which should take about 10 minutes.
